What is CHOICES?
CHOICES is a program designed to help people
who are coping with a serious progressive illness. The program
is also a support program for loved ones as well. Sometimes the
family needs support managing their responsibilities while caring
for a loved one. There are times when a client needs emotional
support and assistance with advanced care planning. And there
are times when a family or their loved one needs help wading through
the medical system.

The CHOICES team is a unique blend of
nursing and volunteers. The CHOICES nurse visits with the CHOICES
client and assists with case managment as well as helping a client
to assess his/her needs in the home or nursing home. Volunteers
give a much needed respite break to caregivers, assist with errands
and provide transportation. Volunteers are valuable in supporting
the client or family with companionship and emotional support.

Patients and families facing serious illness
often have difficult decisions to make. The CHOICES nurse assists
people with counseling about healthcare choices, determining healthcare
preferences and goals. Assistance is available with writing the
health care proxy and more in-depth advanced health care planning,
'The 5 Wishes'. CHOICES also assists with information about home
health services or long-term care. And there is assistance with
gathering information and accessing palliative care and hospice
care. |

CHOICES is an outreach service sponsored
by CAHPC. There is no cost for these services. We have served patients
who have cancer, lung and heart disease, kidney failure, and generalized
debility from illnesses such as dementia and Alzheimer’s. |

Anyone can make a referral to CHOICES. We
are happy to come and talk with groups about the program. We welcome
